1. ACKNOWLEDGMENT AND ASSUMPTION OF INHERENT RISKS
I voluntarily choose to participate in glass fusing workshops, instruction, and related artistic activities hosted at the private studio premises listed above. I understand and acknowledge that glass fusing involves inherent physical hazards, risks, and dangerous conditions, including but not limited to:
CUTS, PUNCTURES, AND LACERATIONS from handling raw, broken, or sharp sheet glass, glass frits, powders, wafers, stringers, and glass scrap.
EYE AND RESPIRATORY EXPOSURE to airborne glass dust, silica powder, kiln wash, fiber paper, or chemical release sprays.
INJURIES FROM SPECIALIZED HAND AND POWER TOOLS, including glass cutters, running pliers, mosaic nippers, diamond hand pads, and wet grinding equipment.
GENERAL PREMISES HAZARDS associated with attending an instructional workshop conducted within a residential/home-studio property.
I confirm that I am in good health, possess the fine motor skills and vision required to handle sharp art tools safely, and suffer from no condition that would compromise my safety or the safety of others in the studio.
2. EXPRESS RELEASE AND WAIVER OF LIABILITY FOR NEGLIGENCE
PLEASE READ CAREFULLY: THIS SECTION CONTAINS A RELEASE OF LIABILITY.
IN CONSIDERATION FOR BEING PERMITTED TO PARTICIPATE IN THE WORKSHOP, I HEREBY EXPRESSLY, KNOWINGLY, AND VOLUNTARILY RELEASE, WAIVE, DISCHARGE, AND HOLD HARMLESS THE INSTRUCTOR/STUDIO OWNER, PROPERTY OWNERS, FAMILY MEMBERS, ASSISTANTS, AND AGENTS (COLLECTIVELY REFERRED TO AS "RELEASEES") FROM ANY AND ALL CLAIMS, DEMANDS, CAUSES OF ACTION, DAMAGES, OR LIABILITIES FOR PERSONAL INJURY, BODILY HARM, PROPERTY DAMAGE, OR WRONGFUL DEATH ARISING OUT OF OR RELATED TO MY PRESENCE ON THE PREMISES OR PARTICIPATION IN THE WORKSHOP, WHETHER CAUSED BY THE ORDINARY NEGLIGENCE OF THE RELEASEES OR OTHERWISE. THIS RELEASE DOES NOT EXCLUDE OR LIMIT LIABILITY FOR CONDUCT THAT CONSTITUTES GROSS NEGLIGENCE, RECKLESSNESS, OR WILLFUL AND WANTON MISCONDUCT UNDER OHIO LAW.
3. SAFETY RULES AND PERSONAL PROTECTIVE EQUIPMENT (PPE)
I agree to strictly follow all safety rules and instructions provided verbally or in writing by the instructor, including:
Wearing ANSI Z87.1 approved safety glasses with side shields at all times when handling, cutting, nipping, or grinding glass.
Wearing closed-toe shoes and fitted clothing made of natural fibers.
Refraining from touching kilns, electrical controls, or designated private areas of the property unless specifically instructed to do so.
I acknowledge that the instructor retains the right to terminate my participation immediately without refund if I fail to observe safety rules.
4. EMERGENCY MEDICAL AUTHORIZATION
In the event of a medical emergency or physical injury, I authorize the host or instructor to seek emergency medical care on my behalf if I am incapacitated. I agree to accept full financial responsibility for any medical expenses incurred.
